At Cornerstone Connections
OUR COORDINATION AND RECOVERY COACHING SUPPORTS ARE KEPT COMPLETELY SEPARATE TO OUR CORE SUPPORTS.
We do not provide both core and coordination to the same customer unless in extremely rare circumstances where this is requested by person we support. In those cases we work with the customer and their supports to find ways to ensure there is no impacting conflict of interest – and we continue to review this regularly. Within our business, our teams do not have access to the same part of the system and their work is not visible to each other. Please refer to our Conflict of interest policy in the Resources page.
The following supports are delivered and in addition we can tailor a support package to you specifically, based on your funding and your needs
Psychosocial Recovery Coaching
With specialised professional and personal experience, our Psychosocial recovery Coaches have a range of tools and strategies for you to use during your time with us and beyond. We have specialist training in Mental Health Recovery support, as well as deep understanding of the NDIS requirements for people with Psychosocial Disability.
Support Coordination
Our Coordination team are here to help you navigate the NDIS, implement your NDIS plan and manage your funding budget. We will explain your plan to you and what your funding is for - and help you find services that you need. We are very experienced working with NDIS processes, and will always have your priorities at the forefront. We keep a small workload for our team to be able to ensure we have the time to support you well and to a high standard. Our support Coordinators are specialised in the area of Specialist Disability Accommodation (SDA), Psychosocial disability and High Physical Support.
Skills training and mentoring
Depending on your specific goals in your NDIS plan, we can provide you gain the skills you need to achieve those! You will work with a dedicated coach/ mentor to make a step-by-step plan and identify programs or other learning opportunities that will help you. These goals may be around getting a job (or a different job), getting your license, moving to your own home or something else ... our team will have the expertise and tools to work with you on those things.
